Nursing Liaisons Mission Statement

“What do you do?” is one of the questions that we get asked most frequently since starting Nursing Liaisons.  We have a mission statement. 

The focus of Nursing Liaisons is to provide compassionate care, guidance, education and support to patients and their families in many areas of their life. As experienced Registered Nurses, we are passionate in believing that we make a difference with each patient.  Our goal is to empower and educate the patients in managing their medical care while providing hope, encouragement and assistance. 

What does that look like?  What do we do? 

Some of the simple answers are we teach American Heart Association, CPR classes.  We help patients that are transitioning from the hospital to home.  We have long term relationships with patients that need assistance on a continuing basis, we advocate for them and help them navigate the medical system.  We provide private nursing care for patients at the end of life.   

An example of what this might look like is one of our patients” Fred.”  We first heard from Fred when he was hospitalized and the recommendation for him was to go to rehab post hospitalization.  He did not want to- he wanted to go home.  He lived by himself with his dog, he had a supportive network of neighbors that are very involved with him.  We helped him go home and stay home with a tailored multi-disciplinary team approach.  The team included the Visiting Nurses, Physical Therapists, Occupational Therapists, Home Health Aides, and the team from Nursing Liaisons.  Fred started to feel better and the services decreased, now with Fred’s input and Nursing Liaisons’ experience, we have a dedicated Registered Nurse visiting Fred four hours per week to help him manage his chronic diseases. “Fred’s nurse” can keep an eye on his medications, diet, appointments, his living situation, and his mental health.  Fred can maintain his independence, live independently, enjoy his dog and his supportive neighbors, and he and his family know that someone is looking in on him and knows him. 

Chronic diseases such as hypertension, heart disease, diabetes, chronic obstructive lung disease-COPD- are managed by the patient 95% of the time. It is very important that any behavior change needed in order to manage or potentially reverse the chronic conditions be a joint effort with the patient as the leader of the change.  It is imperative that the support be available to answer any questions by professionals that know the patient.
