Time seems to be flying by as we continue expanding Nursing Liaisons services to better serve you and our extended community. If you’re unfamiliar with all that we do, I’d like to share some insights into two key areas of distinction. We are a nursing agency that specializes in private duty care and Hospital at Home services. There are some key differences that most people are unfamiliar with, so we’re dedicating this month’s newsletter to help demystify these pillars of care:

PRIVATE DUTY

Private duty care is all about personalization based on the unique needs of each patient and their family. Most often, we assist patients after medical procedures, help with transportation to and from appointments, and provide the most comfortable, home-based care for those nearing the end of life. We deliver custom-tailored services in patients’ homes and hotels. Often our services are provide for a short time, from approximately one day to a three or four days. Our goal is to be the link to the next phase of your care.

We compassionately, serve as a bridge during each of our clients’ healthcare journeys by providing ease, assurance, and the most optimal care.

HOSPITAL AT HOME

We are proud to partner with the Mass General Brigham Hospital at Home program. The Hospital at Home Program provides hospital-level acute care within patients’ homes. Through this innovative program, patients are empowered to choose whether they want to be admitted at the hospital or receive care at home. For the past four years that we have been involved with Hospital at Home, our nurses have shared their excitement about the opportunity to care for patients in their homes, with many commenting on the excellent outcomes and how the experience has allowed them to gain new skills, practice autonomy, and deliver more personalized acute care. Hospital at Home offers a fresh perspective while still maintaining the principles of traditional medicine within the home setting.

Currently, there are over 200 hospital-at-home programs across the United States. The growth of these programs has accelerated, particularly during and after the COVID-19 pandemic. Many hospitals and health systems have adopted this model to provide acute care services in the patient’s home, helping to alleviate hospital capacity issues, reduce healthcare costs, and improve patient outcomes. The Hospital at Home model was originally developed by Johns Hopkins in the late 1990s, but its expansion has been supported by regulatory flexibility, such as the Centers for Medicare & Medicaid Services’ (CMS) “Acute Hospital Care at Home” waiver during the pandemic. The waiver allowed hospitals to receive Medicare reimbursement for delivering acute care services at home.

Hospital at Home is a true passion of ours, and we couldn’t be more excited to see its continued expansion. If you have any questions or are interested in learning more about this innovative program, please don’t hesitate to reach out.
