Nursing Liaisons RNs can ease transitions for you or your loved ones from hospital to home. Our Registered Nurses are experts in transitional care. We can take those complicated discharge instructions, confusing medications changes and sit with you in the comfort of your home and translate them too an understandable, actionable, plan. We can even provide transportation home if it would help!
